During the initial inquiry in regard to admission, the Executive Director determines if a child is appropriate for completing an intake study. The decision to complete an intake study is also based on the both the availability of a placement at the Forest Heights Lodge and also when an admission is possible. If the Executive Director feels an intake study is appropriate, you will be advised of items essential to the process.

The following items should, whenever possible, be received from the referral source or past services to the child.
  • A cover letter describing the child's problems and reasons for seeking placement
  • Psychiatric /Psychological Reports
  • School records
  • Medical reports
All of the above records will be reviewed and approved by the appropriate staff and phone contact may be made with professionals involved in the care of the child. When indicated, additional psychiatric/psychological testing, EEG, neurological, or other specific medical tests or history information may be requested prior to a final decision to accept a child for placement.

Once the intake materials are reviewed and they appear to confirm the appropriateness of the child for Forest Heights Lodge, we will request that the family and child visit the Lodge. While visiting The Lodge, the child and his parents and/or legal guardians shall have an interview and an evaluation. A thorough explanation of The Lodge Philosophy, Discipline & Control Policy and Patients Rights shall be reviewed with the family. If the family and child feel the Lodge is appropriate and the child is deemed by The Lodge to be appropriate, a financial plan will be discussed and agreed upon. In addition, a time, date and other factors in regard to placement are determined and a pre-placement home visit will be scheduled.

We have found a pre-placement visit to the child's home to be a very useful evaluation tool. This provides an opportunity to build the foundation of a cooperative alliance and afford a more detailed understanding of the child's functioning in his environment. In the two or three days we spend living in the home, we can observe the family's interactions, experience how the child relates to his parents and siblings and interview extended family members. The insights acquired from these direct, intimate contacts give us an invaluable head start in planning for the child's treatment. During this visit, we also meet with school and medical personnel as needed, both to clarify the child's past and to begin exploring post-discharge treatment and educational resources.

Professional time and travel cost for the home and school evaluation are billed separately from Forest Heights Lodge's regular monthly cost of care should be paid prior to the admission of the child.
